Policies

PRICES/POLICIES ARE SUBJECT TO CHANGE WITHOUT NOTICE.

CALL FOR QUOTES ON LENSES NOT LISTED IN PRICE LIST.

TERMS:

  • All stock sales net. (No Discounts)
  • Accounts must be current to participate in all incentive programs and special pricing programs.
  • Customer agrees to pay 2% per month service charge on items that remain unpaid 30 days and over.
  • Credit balance is redeemable for merchandise only.
  • Customer agrees to pay all collection fees associated with any unpaid balance.
  • A $50.00 service charge will be assessed for any check returned for any reason.

CREDITS / WARRANTIES:

  • All returns must be presented within 60 days of replacement invoice date.
  • Inspect each order carefully before processing. Lenses subjected to additional processing in your lab indicate that you have accepted the order. This includes tint or edging the lenses.
  • All requests for credit must be accompanied by the original invoice number and remake number with a request for credit form. If our form does not accompany a credit, there will be a $2.00 fee deducted from the credit amount given.
  • All lenses must be returned to P.O.G. in order to insure that your account receives the appropriate credit.
  • In order to receive a credit, you must remake the job with P.O.G.
  • Credits/Warranties will not apply if an account is non-active with our labs.
  • Credits may not exceed 10% of your current monthly gross expenditure with P.O.G. If credits do exceed the 10%, the excess will then be charged back to the account as a debited amount.
  • Credits will be issued within 7 days from when they are received.

GUARANTEED SERVICES:
Orders placed before 2:00 p.m. CST may be guaranteed for a $12 fee. If the lenses are in stock, we will guarantee the order goes out the same day or the lenses are free. Only applies to uncut lenses and not valid on AR coated orders. Orders must be phoned in. Other restrictions may apply. NOTE: To insure accuracy, please call or fax in remakes or changes on previous orders. Do not send remotely.

CANCELLATION / RX CHANGES:

  • Orders cannot be cancelled or changed once they have been placed in process. Rx changes made after work is in process will be charged at regular price and a 25% discount. These charges will be in addition to the original price.
  • There will be no credit issued for patient cancellation.

TELEPHONE / INTERNET ERRORS:

  • All phone orders will be read back and accepted as being correct. Any error will be billed with a 25% discount upon return of original lenses. These charges will be in addition to the original price.
  • Remote orders submitted will be accepted as correct. Any remake will be billed with a 25% discount upon return of original lenses. These charges will be in addition to the original price.
  • Finished stock lens orders will not be accepted or processed remotely. They will be processed at a surfaced lens price.

PROFESSIONAL ERRORS:

  • All phone orders will be read back and accepted as being correct. Any error will be billed with a 25% discount upon return of original lenses. These charges will be in addition to the original price.
  • Remote orders submitted will be accepted as correct. Any remake will be billed with a 25% discount upon return of original lenses. These charges will be in addition to the original price.
  • Finished stock lens orders will not be accepted or processed remotely. They will be processed at a surfaced lens price.

PROGRESSIVE LENS NON-ADAPTS:

  • In the event that a patient is not able to adapt to their progressive lenses, replacement single vision or lined bifocal lenses will be processed at no expense to you within 60 days of the original delivery.
  • Should the patient decide to try a different progressive lens, we will credit the less expensive lens. Lenses must be made in same material.
  • Any original add-on (e.g., tints, coatings, edging, etc.) will incur remake charges.
  • P.O.G. will bill replacement lenses with a notation of credit pending. Credit will be given for the less expensive pair, upon return of the original lenses.
  • Non-adapts will be limited to a one time remake. Professional error can not be used if non-adapt is already used, vice versa.

PATIENT OWN FRAMES / LENSES:

  • Patient’s own/old frames or lenses are accepted for processing only at the patient’s own risk. POF processing failures will be returned to your office along with the lenses, where a similar frame can be selected from your display.
  • P.O.G. is not responsible for any breakages that may occur on lenses sent to be edged down or additional processing (e.g., tint or AR added.)

LENSES:
SCRATCH COATING: Mfg. hard coating is warranted for 1 year, one time replacement.
ZERO SCRATCHES our double sided hard coating is warranted for 2 years, one time replacement.

NOTE: Lenses must be replaced in identical form. A maximum of one replacement per Rx order is allowed. Front surface scratches, due to normal wear, will be covered. However, abuse of the lens will not be covered. Scratch warranty will be approved per P.O.G. discretion. Safety lenses carry no warranty.

DRILLED RIMLESS & GROOVED RIMLESS:

  • Rimless Rxs fabricated in trilogy or 1.60 index and above will be warranted for cracking or breaking for a period of 1 year, 1 time. (Normal wear.) Rxs fabricated in polycarbonate will have a 90 day warranty. P.O.G. does not wish to process lenses for rimless frames in CR39, mid-index, sunsensor or instashades and will carry no warranty.
  • P.O.G. does not recommend polarized lenses be processed in groove rimless frames and will carry no warranty.
  • Based curve selection is based on manufacturer’s specifications. Any special request will incur
    additional charges.

COMPLETE FRAME INFORMATION:
Complete frame information is needed in order for calculations of lens thickness and decentration to be accurate. P.O.G. will not honor credits for lens thickness issues, unless the complete information was given at the time the order was placed. (E.g., frame shape from shape chart, A, B, DBL and ED.) If lenses are being returned for thickness, they must be edged in order to be considered for credit.

ANTI-REFLECTIVE COATINGS:
NOTE:  Applying AR to any lens voids the manufacturer’s warranty and the following policies apply:

IN-HOUSE AR*:

  • Zero Glare plus UV Basic: Is warranted against defects for 1 year, one time. Lenses must be sent in for strip and recoat.
  • Zero Glare plus UV Elite, Zero Glare Clear & Zero Glare DES plus UV: Is warranted against defects for 2 years, one time replacement. Should the coating be found to be defective, the lenses will be replaced. This is limited to the original Rx in the same material and same frame. Customer must return old lenses accompanied by both invoice numbers in order to receive credit.

OUTSOURCED AR*:

  •  Crizal®/Alizé®/Avancé®/Teflon®/Hoya & Kodak AR:
    •Are warranted against defects for 2 years, one time. P.O.G.’s standard credit policies still apply.

*NOTE: P.O.G. will only give credit for defective coatings; We will not give credit for damage caused by improper care. Additional services (e.g., coatings and tints) will be charged at full price. All warranties are at P.O.G.’s discretion.

RETURNS:

  • All semi-finished, stock lens or special order item returns must be accompanied by an invoice number and RA number provided by P.O.G. (A 20% restocking fee will be applied.)
  • All returns are at P.O.G.’s discretion.
  • Special order items ordered that P.O.G. does not carry in our inventory are non-returnable. This includes but is not limited to: frames, special order lenses, contact lenses*, lab supplies, custom clip-ons, lens blanks, etc.
    *Contact lenses carry no warranty.

PACKING SLIPS:
P.O.G. provides customers with packing slips for items that are sent to us. Please enclose a complete packing slip. Upon receipt, we will verify the contents. P.O.G. will not be responsible for any missing items if a packing slip is not enclosed. If sending via UPS, please keep your tracking number for quick and easy reference. Packing slips are also available to print from our website.
P.O.G. provides prepaid UPS labels to send 2 or more new orders. Please do not use these labels for less than 2 orders as a $7.00 fee applies.
You may use our USPS prepaid mail labels to return lenses for credit or less than 2 new orders. Labels can be printed from our website.
